Due to the departure of our POH-S from 1 May 2025, we are looking for an enthusiastic, decisive and pleasant new colleague who would like to fill this position.

What will you do?

As a POH-S in our practice, you will supervise patients with Diabetes Mellitus and Asthma/COPD. You provide information, advice, education and instruction. You motivate and encourage patients to adopt a healthy lifestyle and support them in coping with their illness as well as possible. Positive Health is an integral part of this. You have your own consulting hours and take care of all patient and quality assurance tasks associated with the POH-S position.

You are patient-oriented, committed and have an affinity with the target group. You get energy from motivating and supporting patients, paying attention not only to the medical aspect but also to their social well-being. You take initiative, set priorities effortlessly, work independently and are a real team player at the same time. In addition, you are empathetic, sociable and flexible - in short, a valuable colleague within our team. You also bring:

• A completed course in POH-S or an HBO nursing degree with ambition to pursue POH-S training.
• Working experience with patients with chronic conditions, such as cardiovascular disease, is a plus.
• Knowledge of insulin therapy, a Caspir diploma and experience with spirometry are a plus.
• Skill in using ICT.

Meet the practice

Huisartsenpraktijk Westeinder is a modern, NHG-accredited, general practice in the heart of Aalsmeer. Close to the nice centre and near the beautiful Westeinderplassen (lakes). Our team consists of four GPs, five assistants, two POH-S, a POH-GGZ, a POH GGZ youth and a practice manager. We are a low-threshold practice and like to work with great attention to good, person-centred care.
